Omega IV was a Class M planet that inexplicably was home to an apparent Human civilization consisting of two groups: Kohms and Yangs. The Yangs were apparently descended from Yankees, complete with exact copies of the Bible, United States Constitution, and the flag of the United States of America. The Kohms, likewise, were apparently descended from Asian communists. Further, the cataclysmic biological warfare that devastated the planet occurred nearly 600 years earlier on Omega IV than the averted biological warfare of the late 20th and early 21st centuries on Earth.[1]

In 2268, the U.S.S. Exeter NCC-1672 visited the planet. Unfortunately, the diseases unleashed centuries earlier still lingered, causing the deaths of the entire crew; only Captain Ron Tracey was spared, as he remained on the surface, where local conditions provided immunity. Tracy proceeded to violate the Prime Directive by providing phasers to the Kohms. When the U.S.S. Enterprise NCC-1701 arrived six months later, Tracy killed Ensign Galloway before he could be taken into custody by Captain Kirk.[1]
